iOS controls when an app may play sound in the background. A notification tone is reliable; a full Azan playing unattended may not be, especially after a restart.

Sleep and Focus modes can suppress the very notification you need at Fajr. Allow the app explicitly in the relevant Focus.

No iPhone configuration guarantees audio at a precise moment indefinitely. If the household depends on hearing the call, a dedicated device is more dependable.
